Project Personnel
- Definition: Includes research assistants, full/part-time assistants, temporary workers, etc.

According to Article 16 of the "Labor Health Protection Regulations," personnel hired for 6 months or more must undergo a general physical check-up before starting work.
- Under 40: Provide check-up report within the last 5 years.
- 40-65 years: Provide check-up report within the last 3 years.
- 65+ years: Provide check-up report within the last 1 year.

Plan personnel must submit a physical examination report to the Environmental and Occupational Safety Center (1st floor, Cheng Ai Building) before completing the application in the "General Affairs Accounting Integration Management System - Project Personnel Management System."
- Only after submission will the application be reviewed and approved.
- Ensure the report includes all required items; missing items will lead to rejection.

Please visit the affiliated hospital or other recognized medical institutions for labor health examination.
- Examination items (based on "Labor Health Protection Regulations, Schedule 9"):
- Survey of work history, medical history, lifestyle habits, and symptoms.
- Height, weight, waist circumference, vision, color discrimination, hearing, blood pressure.
- Physical examination and inquiry of body systems.
- Chest X-ray.
- Urine protein and occult blood test.
- Hemoglobin and white blood cell count.
- Blood glucose, ALT, creatinine, cholesterol, triglycerides, HDL cholesterol tests.
- Other tests designated by the central competent authority.

Fast for 6-8 hours before the check-up. Bring ID and NHIC card (no appointment required).

Check-up time: Monday to Friday, 9:00-11:30 AM and 1:30-4:00 PM.
